What is Share the Vision Referral Program?
Our team at Family Optometry Center of Orange prides itself in the exceptional services we provide our patients. It is our continued desire to extend this service and provide your friends and loved ones with the same experiences that have won us your loyalty. Our Share the Vision Referral Program is designed to reward you for your continued support as you refer others into our care.
What are the benefits?
- 1st new patient referral: $20 Certificate
- 2nd new patient referral: $20 Certificate
- 3rd new patient referral: $20 Certificate
- 4th new patient referral: $30 Certificate
- 5th new patient referral: $30 Certificate
Each new patient you refer will be rewarded a $20 certificate.
**Referred patient MUST receive an exam.
**No more than one certificate can be awarded per referred household.
What can the Referral Certificate be redeemed for?
The Referral Certificate can be used toward:
- Eyewear
- Annual supply of contact lenses
- Other eye products (e.g. supplements, eye masks)
How are Referral Certificates tracked?
When a referred patient comes to their first appointment they must present a Share the Vision Referral Program Voucher to front desk staff. Once received we will e-mail you a credit notice. Your credit will be noted in your chart and MUST be used within 1 year of notice.
Can the Referral Certificate be combined with insurance?
The Referral Certificate does not cover services or insurance co-pays.
** Certificates do not have any monetary value.
Is the Referral Certificate transferable?
The Referral Certificate is not transferable between patients.
How many Referral Certificates can be earned?
There are no limits as to how many patients you can refer. However, there is a limit of 5 Referral Certificates ($120 value) within one year of your last exam date.
